Inpatient Care Management

Our care management team provides services aimed at coordinating your hospital care and discharge plan, as well as transitioning your care back into the community. This includes communicating with your insurance, working directly with your nurses and physicians to assure your needs and concerns are being addressed, coordinating immediate follow up care needs, arranging appointments with your primary care physician or specialist and referring your care to the outpatient care management team or other community services and programs for further support and disease management.
