What are 'Medicare Face-to-Face' requirements?
For certain specified items of durable medical equipment, the Affordable Care Act requires:
- An in-person, face-to-face examination with the treating practitioner (Medical Doctor (MD), Doctor of Osteopathic Medicine (DO) or Doctor of Podiatric Medicine (DPM), physician assistant (PA), nurse practitioner (NP) or clinical nurse specialist (CNS).
- The treating practitioner must document that the beneficiary was evaluated and/or treated for a condition that supports the need for the item(s) of DME ordered.
- The face-to-face examination must have occurred sometime during the six (6) months prior to the date of the order for the item.

- For urgent situations requiring an immediate response, call 301-714-0200, and follow the voice message instructions for contacting the on-call representative.
- If there is a life-threatening situation or a medical emergency, Call 911.
- In the event of an environmental emergency, such as inclement weather, natural disaster or industrial disaster, and you are unable to reach us by phone, tune to your local radio or television station for instructions on how to reach us.
- Your physician is the only member of the health care team who can diagnosis or prescribe. If you have questions about your prescription or diagnosis, please contact your physician.

Do you bill insurance?
Meritus Equipped for Life will bill your insurance for covered items. Any co-insurance, deductibles or non-covered items will be your responsibility. Please see below for additional helpful information.
Medicare:
- Medicare will pay for home medical equipment, also called durable medical equipment, at 80% of the Medicare reimbursed amount as long as you meet the Medicare requirements.
- You are responsible to pay your annual Medicare Part B deductible and the 20% co-insurance for services rendered unless you have a secondary or supplemental insurance that will cover the remaining balance.
- Medicare requires an order for every item of durable medical equipment, prosthetics, orthotics and supplies. Some items may require you to be seen by your physician for a face-to-face examination. Some of the most common items are hospital beds, manual wheelchairs, pneumatic compression device, nebulizer, CPAP machine and supplies, TENS unit, and some types of oxygen.
Medicaid:
- Medicaid pays home medical equipment at 100% as long as you meet the medical need requirements. Some equipment may need to be pre-authorized prior to receiving.
- Medicaid requires an order for every item of durable medical equipment. Medicaid requires that we follow the medical policies of Medicare for coverage determination.
Private or commercial insurance:
- Your insurance company must approve home medical equipment.
- Your insurance company may not pay 100% for medical equipment.
- You will be responsible for any ESTIMATED co-payments upfront. Any remaining balance will be billed to you once your insurance has finalized payment of your claims.
- Your plan may have a limited amount of coverage or benefit exclusions on some home medical equipment.
- Your plan may have a limit on the types of services it will cover.
- The delivery ticket you sign verifies your agreement to accept responsibility for any cost not covered by your insurance company.
- All of this will be determined at the time your insurance coverage is verified. Information regarding coverage will be communicated with you verbally and in writing prior to the delivery of equipment.
Self-pay:
- Any patient with no insurance (Medicare, Medicaid or Private/Commercial) is initially considered as self-pay.
- If you have no way to pay for your services, please contact our office to discuss potential financial options available to you.
Payment options:
- Payment may be given to staff during the delivery of equipment.
- Payment can be mailed back in the return envelope included with your bill.
- Payment can be made over of the phone by contacting our billing department at 301-714-0200.
