CARE CALLERS

Loneliness and social isolation have been shown to impact life expectancy as much as smoking, obesity and physical inactivity. Meritus Health has set out to make sure that loneliness doesn’t take its toll on our community… One phone call at a time.

Care Callers is designed to help Meritus patients who answer they feel a lack of companionship or feel lonely on the social determinants of health questionnaire asked prior to an office visit. The program has a goal of 50% of enrollees reporting they are less lonely within 4 months of engagement with program intervention. (Within four months of being in the program, 35 out of 37 people say they are feeling less lonely.)
