Our Services

Services are available seven days a week and include the following:

  • Patient Assessment to evaluate your personal as well as family and home situations.
  • Discharge Planning coordinates the care you will need after you leave the hospital. This may include home health care, medical equipment, nursing home placement, or assisted living facility placement.
  • Information & Referral to provide information about community services, such as Meals on Wheels, support groups, and Maternal Child Health services.
  • Counseling to listen and help ease your concerns about emotional, social, and financial issues.
  • Assistance in completing advance directives, living wills, and durable medical powers of attorney is also available.
  • Emergency Department Care Management - Many patients come to the emergency department (ED) with the thought that their condition might require admission to the hospital, but not all patients need this level of care. Emergency department care managers work with emergency medicine professionals to evaluate patients based on medical necessity and place patients in the correct level of care. As registered nurses, our ED care managers educate patients on disease prevention, disease management and access to primary care physicians and community resources. Care managers also arrange outpatient services and make follow-up physician appointments. For many of our ED patients, care managers connect the dots to public health, primary care, specialty care, acute care, long-term care and urgent care resources.

Discharge Advocacy Center- At Meritus Health, we believe in preventative care. Nurses from the Discharge Advocacy Center explain post discharge test results and direct patients to the appropriate level of care. Nurses from the Discharge Advocacy Center call patients to ensure that discharge instructions are understood and being followed, check for any changes in patient conditions, explain post discharge test results and make appropriate follow up care appointments. In some cases, our nurses identify new conditions from lab and tests results such as elevated A1C levels, pneumonia and cardiac conditions—or discover urgent situations like rapid weight gain, shortness of breath and allergic reactions to medications. In short, our nurses act as a safety net and continue to assess how the patients' care and health is being maintained and improved once they leave the hospital.
