Heart Disease Management Programs

Cardiac Rehabilitation

The cardiac rehabilitation program is a lifestyle modification and rehabilitation program for patients recovering from a heart attack, coronary bypass surgery, valve repair or replacement, balloon angioplasty and for heart failure patients who meet certain criteria. Three days a week, a cardiac nurse and exercise physiologist work with you to develop exercise plans based on your heart condition, physical ability and lifestyle.

During the 12-week program, you can also participate in educational classes on medication management, a heart-healthy diet and stress management.

Our program is nationally certified by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR). Most insurances companies including Medicare cover the cost of cardiac rehabilitation. For more information, call 301-790-8940.

If you’ve been diagnosed with congestive heart failure, our program will help you identify changes in your condition to prevent future hospitalization.

Each week, an automated system calls you at a specific day and time of your choosing. The voice-activated system asks you a series of questions regarding your heart status, such as current weight, symptoms and medication changes. This information is sent in real time to a registered nurse who reviews the data and looks for any responses that may indicate a change in condition.
